   Getting the spine parallel to the table is a guess with limited success. You may be able to use a straight tube for a solidly built person, but for many, the hips are wider than the waist so a down tilt is required. I go one step further and posturally position the spine by raising the shoulder and lowering the hip they are lying on. Palpation of the spine will determine the angle required. That is also how to get perfect thoracic spine laterals as well. To turn radiography from a trade to an art form, you will need to practice radiography without any entering points. To do that, accurate collimation is required. Using the illuminated field determine what you want on the top or the bottom of the image. For a lumbar spine, put the sacrum on the bottom and ensure the symphysis pubis does not get a dose. That is for the AP. For the lateral, you need to know from looking at images what proportion of the exposed field relates to the palpated pelvis and where the sacrum lies.
